We are happy again to offer Pizza Thursdays from Plum Tomatoes by La Sorrentina every thursday this school year for grades K-4. This form will cover every Thursday until Thursday 2/17 for Grades K-4.  


Every Thursday for the dates indicated below, students have the option to purchase pizza for lunch.
Each slice is $2.50.
Students in Kindergarten can order one slice (Please note that the slices are cut in half for the children to make it easier to eat)
Students in Grades 1-4 can order up to two slices.

11 Pizza Days
 
November: 4th and 18th
December: 2nd, 9th, and 16th
January: 6th, 20th, and  27th
February: 3rd, 10th, and 17th

NOTE:  Please fill out a SEPARATE order form for each child.

After you submit your order you will be charged on your FACTS account. Payment is expected within 20 days of posting.

The completed form(s) is due by Friday 10/22.

No refunds will be issued if a child is absent.
