I, for myself, my child/children or ward/wards sign this Waiver and Assumption of Risk in consideration
of the opportunity to use the facility, or to participate in any parties or activities at/by KIDZ AT PLAY
INC.

I, for myself, my child/children or ward/wards acknowledge and understand that there are dangers and
risks associated with the activities at/by KIDZ AT PLAY INC and agree to assume all risk of personal
injury, including the potential for paralysis and death.

I, for myself, my child/children or ward/wards agree to follow the safety instructions provided and
acknowledge that failure to do so may result in expulsion from KIDZ AT PLAY INC.

I, for myself, my child/children or ward/wards, and on behalf of my or their heirs, assigns, personal
representatives and next of kin, HEREBY HOLD HARMLESS KIDZ AT PLAY INC, its owners,
members, officers, employees, equipment manufacturers and sponsoring agencies from all liability for
any such personal injury, disability, death or loss or damage to person or property to the fullest extent of
the law.

• Children must be accompanied by an adult who must remain on the premises at all times.
• All guests are required to wear socks, children must wear non-slip (grip socks). Signing
• this waiver acknowledges that you are confirming your child or children are wearing
• non-slip socks.
• BE SAFE. No Running. No climbing on the outside of the play structure nets.
• BE SAFE. Always slide down feet first. Do not climb up the slides.
• BE SAFE. Pushing, rough play and shouting will not be tolerated. The staff reserves the right
• to ask the guest to leave.
• No Food or Drinks allowed on the playground area.
• CHILDREN MUST BE SUPERVISED AT ALL TIMES. This includes the lounge and
• washrooms.
• SICK CHILD POLICY: As a courtesy to our staff and guests, children who are sick
• (sneezing, coughing, fever, visibly unwell) should not visit the facility until they feel better.
• ALCOHOL IS NOT PERMITTED
• Toys are to remain the specific play area
• No toys are permitted on the play structure.
