Mobile Device Agreement Acknowledgement Page
By signing this acknowledgement page, you are confirming that you have read the “Student and Parent Agreement for Chromebook Use 2019-2020” and the “Monroe County Acceptable use of Technology” policies and agree to all its terms and conditions. The policies listed above can be found at https://www.sigsbee.org/families/policies. I further understand that the term “chromebook” will also refer to an “iPad” with regard to this acknowledgement. I also understand that a hold charge will be placed on my account for the replacement cost of each device. The replacement costs are $408.00 for an iPad and $229.00 for a chromebook. The charge will be dropped when the device is returned. Any lost of broken charging devices will be assessed a $25.00 charge.

Acknowledgement: Student and parent/guardian acknowledge that they have read the “Student and Parent Agreement for Chromebook Use 2019-2020” and the “Monroe County Acceptable use of Technology” policies, understand them, and agree to be bound by their terms and conditions. Student and parent further acknowledge that this agreement represents the complete understanding and agreement between the school board (Sigsbee Charter School) and the parent and student with respect to the subject matter hereof. No other representations, stipulations, agreement or understanding, whether oral or in writing shall be valid or enforceable or have binding effect unless contained in this agreement. This agreement may not be changed, amended, or modified without the express written approval of the school board (Sigsbee Charter School). Any change, modification, or amendment to this agreement approved by the school board (Sigsbee Charter School) must be in writing.
