We, the Participant, understand that we are responsible to act in a safe and responsible manner, and to obey requests to comply with safety regulations as directed by the designated leaders. We will be responsible for ourselves when going to or from activities. We will not endanger the safety of ourselves and others at any functions. We forsake all rights to sue Westside Baptist Church, her pastoral staff, deacon board members, leaders, staff or members in the event of physical or emotional injuries, including death. We agree that the leaders can bring us immediately to a medical centre for treatment in case of emergency, AND WE ARE RESPONSIBLE FOR ANY COSTS ASSOCIATED WITH THIS TREATMENT.

I hereby give my consent to the use of my testimonial and physical likeness in photographic, video or electronic reproduction form in any materials, and on websites. I release Westside Baptist Church, its agents, staff and volunteers from liability for any violation of any personal or proprietary right in connection with such use. I waive all rights to royalties or other compensation arising from, or related to, the use of the photograph and/or testimonial. I may request access to view and/or obtain a copy of my testimonial or physical likeness. I realize that I may have to pay the costs of having a copy made. I will not make copies of them without written approval of Westside Baptist Church. I understand that I have the right to revoke the permission any time by contacting Westside Baptist Church. This decision will not have a negative effect on the services I receive from Westside Baptist Church.
