Request to attend Chronic Pain Anonymous Service Board (CPASB) monthly meeting.
During a fellowship Open Forum, an idea arose to have members of the fellowship attend CPA Service Board  meetings and monitor the meeting. 

Requests will be considered on a first come basis. In order to be considered to attend, the following form must be completed by the third Monday of the month. Up to three members per month will be able to attend. You will be notified via email of your request status. 

Meetings are held via Zoom on the fourth Tuesday of the month at 4:30pm ET.
