Fill out this form to make sure you receive your make-up classes from your teacher.

Only include summer vacation time in the months of June and August.

There are no regular classes in July and you do not pay tuition during that month - you will not receive make up classes for any time you miss during that month.  

Miss 3 Classes = 1 Private Lesson + 3 Make Up Group Classes
Miss 4 or 5 Classes = 2 Private Lessons + 4 or 5 Make Up Group Classes
Miss 6 Classes = Make special arrangements with your teacher

The office will notify your teacher and your teacher will schedule your private sessions. If you do not qualify for a free private session, the office will let you know.

If you are a PRIVATE STUDENT or take PRIVATE LESSONS, you arrange your private make-up classes directly with your teacher. Do not fill out this form for Private Lessons.

MAKE-UP GROUP CLASSES are scheduled through the office when you return for vacation. When you return from vacation, just call the office and we will help you.
