The Participant (and the Parent/Guardian) understands and agrees that participation in any Force Sports activity is completely optional (NOT MANDATORY) and is at your discretion and is your decision.  If you are not comfortable at any time, it is your responsibility to not participate and/or to remove yourself from any situation you choose. *
ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENTIn consideration of being allowed to participate on behalf of Force Sports and all of its affiliates including, but not limited to, all facilities, clinics, training, club/travel teams and all athletic programs and related events and activities, the undersigned acknowledges, appreciates, and agrees that:    1.  Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; The risks of injury and illness (ex: communicable diseases such as MRSA, influenza, and COVID-19) from the activities involved in this program are significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce these risks, the risks of serious injury and illness do exist and,    2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,    3.  I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,    4.  I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Force Sports (and all affiliate programs) their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law. *

FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)   This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.

Force Ohio Tryout FAQ'S
How many tryouts do I have to attend?
 
Players only need to attend (1) tryout. They may choose to attend multiple tryouts, however, only (1) is required.
 
Which tryout should I attend?
Players trying out for a particular side of town are asked to go to that side of town for tryouts (Eastside or Westside).
 
When should I arrive at tryouts?
 Players will need to arrive at least 15 minutes prior to the scheduled start time of their selected tryout.
 
When should I bring to tryouts?
All players are responsible for bringing and using their own equipment: bat, glove, spikes, etc and water/sports drinks.
 
What happens if there is inclement weather?
If there are any changes to the Tryout schedule due to inclement weather, updates will be posted on Twitter: @ForceBaseball1 and on the home page of www.force-sports.com
 
How do I know if I made the team?
Players will be notified if they make the team via phone call and/or email in the following Days after the Tryout. If a player does not receive a call/email, please wait until they do so as there are many evaluations we have to go through from multiple tryouts and want to make the best decisions for each roster.
