This form is required for the Cynthia Cooper Memorial Scholarship and the Mozart Scholarship applications. Mozart Scholarships may be awarded multiple years for qualifying students.
This form is NOT required for the Outstanding Performance Scholarship.

We want to encourage deserving students who will benefit from the opportunity of attending the Houston Summer Harp Festival. Please submit by May 20th at midnight.
