Do you intend to have your child enrolled in our program for the entire school year? If not, please explain why. School year is from August 22nd to June 10th, January session from January 3rd to June 10th *
What school experience has your child had? I.e daycare, nursery school, church program, story time at libraries, gym classes, etc. *
Is your child fully potty trained. Meaning they do not need any assistance going to the bathroom, will say when they need to go to the bathroom, does not need to where pull-ups or diapers, is free from accidents.  Children who are determined to not be fully potty trained will be dismissed from the preschool program. *

Do you understand that the monthly tuition is due on the first of each month. Failure to pay by the first, will add a late payment fee of $5 for each day. If payment is not received by 5th day, child will not be allowed to return until payment is received. After two weeks child is dismissed from the program. *
