Class of 2023 Senior Form                           Graduation: May 19th  7pm  @ Menard Center
Upon completion of your last class, please complete the following form to help us get you that much-deserved diploma!  Graduation Date is subject to change. Please make sure to include good contact information so we can let you know about graduation or if you choose not to walk we need to know where to send your diploma.

In the event of virtual graduation, or you choose not to walk at the ceremony please provide us with a good mailing address for your diploma.
Please email Mrs. Kemp a current picture of yourself (Senior picture or one that is school appropriate) and a younger/baby picture of your self.  These will be used during the graduation ceremony.  natasha.kemp@matsuk12.us  You may also drop this off in-person at the front office and we will return it to you.  The deadline is April 1st!
At the graduation ceremony, your name will be read along with what your plan for after high school.  For example:  Billie Blue plans on going to AVTEC to study welding.    Bobby Jones plans on traveling for a year before gaining a job as a CNA.        Now please tell us what your plan for after high school will be.  We can always update your plan at graduation practice. *
Choose the closest size to your height for your graduation gown.   You can purchase your own gown to wear through Jostens.   Go to Jostens.com *
This year we will have a special slide show that will show during our graduation ceremony.  Please write a short thank-you to someone that has helped you achieve your graduation goals. (This could be a parent/guardian, teacher, etc) *
If you plan on joining a branch of the military after graduation  please tell me which branch.
If you have been accepted to a University and/or trade school or have been granted a scholarship please list below.
