Recreation Activity Sign Up
So that your family can be sure to get all the wanted rec activities in while you are at camp, please sign up for the options you want enjoy below.  Your selections will be pre-scheduled prior to your arrival.  Please note any families with whom you would like us to try and combine your rec options.  If your group exceeds the maximum number of participants we can serve at one time, you will be scheduled for additional times.  Unless otherwise noted, all options are included in your program fee.  
Please let us know which families at camp you already know and would like to share rec activities with when space allows.  NOTE - They will sign up for their own rec activities.  Please only sign up YOUR family for rec activities!

# of people who wish to go Paintballing -Cost of $20 per person, minimum age of 10, those 12 and under must also have a parent participating - Will require off-site travel.

Are you bringing a 1st time family to Family Camp in 2022?  Please tell us the name of the primary contact for the $200 Spark Credit.
I give my permission for my family to participate in all aspects of the camp's program except as noted.  I understand an effort will be made to contact me if a family member needs emergency medical-surgical treatment.  I hereby give permission to the staff member selected by the camp to secure proper treatment, hospitalize, order injections, anesthesia, x-ray or surgery as deemed necessary for the adult(s) and child(ren) named above.  I accept responsibility for payment of such services. I will in no way hold Ingham Okoboji, staff members or board members liable.  I give my permission for any picture or video taken of my listed family members to be used for promotional purposes.  I understand that choosing to participate in the program may increase my family’s risk of being exposed to COVID-19 and agree to pre-screen all family members before arrival, not bring anyone who is sick to camp, and honor social distancing guidelines as needed for the safety of myself, my family, other campers, and IOLBC staff.   *
