The 2019 MERIThon happens Monday, July 1st.  This form is for TEAM CAPTAINS to complete. Once completed, you will receive an automated email confirming your responses. If anything changes after you complete the form, a link in that email will bring you back to this page to edit your responses.

You will also receive an email from dan@runtobeer.ca (not quite so quickly) when your form has been reviewed. Once verified, you (the Captain) will be responsible for submitting your team's entry fee.
