Please accept my registration for the JKMBA Basketball activity. I understand that any unsportsmanship behavior may result in suspension from participation. I agree to all of the following terms whether or not this application is signed. MINOR WAIVER AND RELEASE OF LIABILITYIn consideration for my child being allowed to voluntarily participate in any way in the Joseph Kessler Memorial Basketball Association Basketball (JKMBA) league, camps, clinics, lessons, open play, related activities and special events the undersigned hereby acknowledges and agrees: 1. THAT I HAVE READ AND UNDERSTANDS THE CONTENTS OF THIS WAIVER AND RELEASE OF LIABILITY. 2. I agree, represent and affirm that the minor participant is in good health and good physical condition and does not suffer from any disability that would prevent or limit their voluntary participation in basketball, leagues, clinics, lessons, open play, related activities and special events. 3. I agree that the I, as parent(s) and/or legal guardian(s), will instruct the minor participant that prior to participating, he or she should become or be familiar with the operation of, and inspect the facilities and equipment to be used, and if the participant believes anything is unsafe, he or she should immediately advise his or her coach or supervisor of such condition(s) and refuse to participate. 4. I acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses might result not only from their own actions, inaction, or negligence, but the action, inaction or negligence of JKMBA, its employees, agents, directors, and owners, and others, or the condition of the premises or any equipment uses. Further, that there may be other risks not known to anyone or not reasonably foreseeable at this time. 5. I assume all the foregoing risk and accept personal responsibility for any damage or loss following injury, permanent disability, death or loss of property. 6. I RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE JOSEPH KESSLER MEMORIAL BASKETBALL ASSOCIATION (JKMBA), their respective administrators, directors, agents, coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of the premises, all of which are hereinafter referred to releases, from any and all liability to each of the property, caused or alleged to be caused in whole or in part by the negligence of the releases, or otherwise. I/WE HAVE READ THE ABOVE WAIVERS, RELEASES AND JKMBA INFORMATION AND UNDERSTAND THAT I/WE HAVE GIVEN UP SUBSTANTIAL RIGHTS OF THE MINOR PARTICIPANT BY SIGNING IT, AND SIGN IT VOLUNTARILY. *
