This form should be completed by attorneys currently on a CAPDS panel and seeking admission to additional panels. Those currently on the panel but not seeking additional panels should complete the Retention Application. Those not currently on a panel should complete the New Application for the CAPDS Panels.

Understanding the Application Process

Before beginning, you should gather the following:

A list of your most recent and/or serious jury trials including defendant, cause, charge, court, date, outcome, etc.

A list of your most recent and/or serious mental health cases (if applying to a Mental Health Panel)

List of recent appeals (if applying to an Appellate Panel)

Caseload summaries.

Caseload dispositions for 2024.
