Please indicate the number of medallions your area will require for your Finals. CPF-NB will provide a gold, silver and bronze medallion for each category in which you are participating at the Provincial Finals (maximum of 16 gold, 16 silver, and 16 bronze).

If your area would like to purchase extra medallions, they are available from CPF-NB at a cost of $7.00 each, plus taxes.  Please include a cheque for any extra medallions ordered.

The medallions along with the invoice for shipping and handling will be forwarded to you as soon as we receive this form. Please allow time to process your order.

Please submit this form at least two weeks prior to your Area finals to allow time for shipping.
Shipping and handling fees will apply.
