Spring 2023 Culverhouse LIFT Registration:  Youth Tutoring
This is for members of the community who wish to register for our free youth tutoring sessions.  There is a separate form for these classes:  QuickBooks and Computer & Phone Skills.  Go to lift.culverhouse.ua.edu.

Enter the student's name that needs tutoring. *
Select the grade level of the student. *
Enter the subject(s) for which tutoring is needed.   For high school subjects, be specific, such as Algebra II or Chemistry.  We will attempt to find a tutor for advanced subjects, but we must know ahead of time.  Space is limited.   *
Enter the parent or guardian's name. *
Enter the parent or guardian's email address.  We are not able to communicate directly with minors; we will communicate with the guardian.  We will confirm your registration and provide more details using this email address.  You will need to confirm your registration within 3 days of receipt to reserve your space. *
Enter the parent or guardian's cell phone number. *
Select the YOUTH TUTORING section you wish to register for.  Once a class is full, we will remove it from the list or add it as a "wait list" option.  We often have people that do not show up or change their mind, so we will contact you if that happens.  If you are on the wait list, please do not come unless you are contacted.  Please note that the location of evening tutoring has been moved from Hasson Center to Northport Community Center.  Also, the Thursday 6pm time has been changed to Wednesday 6pm. *
To ensure we provide quality tutoring and that we operate the program in an efficient manner, we have policies and procedures that must be followed by all parties.  Please acknowledge that you have read each policy below and agree to the terms by clicking on each box. *
