About:
This survey is part of a minor project research into designing a muscular recovery tool, being developed at Nottingham Trent University.


Important Information:
You must be aged 16 years or older to participate in this survey.

You will be asked to provide your initials, followed by your birth day and month (II/DD/MM). For example:

Chris Bold, 5th February = CB0502 = Your unique code!

This will be your unique code that only you will know! If you wish to reject your participation in this study, then please contact the investigator with the email below, followed by your unique code.

By participating in this survey, I give my informed consent to participate in the study run by Connor Strudwick and Nottingham Trent University.

All collected results will be stored on a private computer in an encrypted folder, which only the investigator has access to.
