Returning MAYHEM athletes: If you need a new size for your uniform, or if would like to order a yellow
uniform with a thicker black outline of the white number, please complete the
order form below and submit payment. You are also strong encouraged to try-on the Warm-Up shirts before submitting your order (same as Rothman 1/4 zips in 2021-2022). Your
uniform number may not be changed.
NEW MAYHEM athletes: Each NEW athlete is required to try-on,
order, and submit payment for a uniform and warm-up shirts. ALL uniform orders MUST be submitted by August
1st so that we can receive our uniforms with plenty of time before November
competition.
