MIB Hockey Participant Waiver

In consideration of being allowed to participate in any way, in any events hosted by FCAC Enterprises, LLC dba MIB Hockey (MIB Hockey) I acknowledge, appreciate, and agree that:

1.      The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

2.      I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown of my participation in MIB Hockey events, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

3.      I willingly agree to comply with the stated and customary terms and conditions for participation in MIB Hockey events. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest staff member immediately.

Terms & Conditions
The applicant, on behalf and for the benefit of the participant(s), hereby applies to MIB Hockey, for participation in its programs. The applicant understands and hereby acknowledges that there is the risk of serious and permanent bodily injury to the Participant(s) as a result of participation in any sporting and athletic contest, including skating. By signing this form, the Applicant and the Participant(s) agree to assume the risk of serious and permanent bodily injury as a result of participation in the programs. The Applicant and Participant(s) further agree to obey and abide by all rules and regulations of MIB Hockey and forfeiture of all fees. No refunds will be issued for withdrawal from any program at any time for any reason.
Release from Liability
In Consideration for the privilege of participation in MIB Hockey programs, the Applicant for himself/herself, and for the participant(s) and on behalf of any heirs, assigns, personal representatives and next of kin, hereby expressly acknowledge the risk of serious injury from participation in this program, and RELEASES AND HOLDS HARMLESS MIB Hockey, their Owners, Board of Directors, Officers, Employees, Coaches, Sponsors, Committee Chairpersons, Volunteers, and any Affiliate Companies, from any and all claims, of any type whatsoever, for bodily injuries sustained by the Participant(s), or the Applicant, arising out of, or from any MIB Hockey events or other sponsored activities.
Waiver/Photograph/Video Release
I authorize MIB Hockey to use photos, and or video of myself, participant(s), and or my child for whom I have legal guardianship for any promotional materials regarding MIB Hockey programs or services. Signing this release signifies that the participant(s) have read, understood, signed, and agreed to the MIB Hockey Video Release Form.
