This form is to be completed when a student, visitor or staff member have sustained an injury requiring first aid ON THE ACADEMY SITE or during an ACADEMY TRIP.   If the injury is as a result of a dangerous occurrence or near miss, please also complete the Incident or Near Miss Form.
