TO REGISTER FOR ONE OF OUR LIMITED EVIA SLOTS
1. Fill in Google Form and click "Submit".
2. Wait for an email for the confirmation of your slot.
3. After receiving the email confirmation, secure your slot by depositing a Reservation fee of P25,000. This is non-refundable and non-transferrable, but fully deductible from your last payment of any tuition payment plan. Your slot will be held for 48 hours only from the time TLA emails the confirmation, so please monitor your inbox. Proof of payment, and all future payments, should be sent to:

Subject Title format: RF, Plan Number, Program Name, Last Name, First Name
(Example: RF, Plan 4, Kinder, Potter, Harry)
After verification, an acknowledgement of your payment will be sent via email.
* The auto response of your Google form submission IS NOT the “emailed confirmation”. Please wait for an email that clearly states what program and time slot are available in exchange for your reservation fee. *
4. Complete your enrollment on or before May 31, 2023 by remitting to TLA Evia all remaining payments, Post Dated Cheques (PDCs), if applicable, and other pertinent documents. All requirements must be submitted by the due date to ensure your slot. TLA reserves the right to re-open your slot to others if the requirements are not fulfilled. The "no refund" policy applies.

a. Pictures of Student - 6 pcs. (size 2x2)
b. Recent family picture - 1 pc. (size 4R or 5R)
c. Copy of PSA-issued Birth Certificate
d. Copy of Immunization/Vaccination Record, including Covid-19 vaccines (if applicable)
e. Agreement Form for Deferred Payment Plans (if applicable)
"FIRST COME FIRST SERVED BASIS" This form will serve as your ordinal proof of reservation.
