We are delighted to welcome your daughter to Batley Girls’ High School. We are committed to working closely in partnership with you to ensure your daughter is happy and successful with us.

For this reason we are asking for contact details from you which are essential for school to hold, to ensure in the case of an emergency or should we need to talk to you about your daughter's welfare, we are able to get hold of you.

This form may take up to 10 minutes to complete. We ask that you only submit your form once (unless you have more than one child joining us in September 2020, for e.g. twins. If this is the case, please complete a separate form for each child).

As part of this document, we will ask for the names and addresses of all adult(s) with parental responsibility, this is information we are required to collect by law. It is particularly important that we have a mobile number and an alternative contact number, together with the names of two emergency contacts. The mobile number provided for priority contact one will be linked to our school app and text messaging service to keep you updated about school closures, snow days and other important information.

We also understand that sometimes other family members may act as a representative on behalf of your daughter and it is important we know you are happy for school to contact these people either in an emergency or to discuss your daughter’s welfare. We are therefore asking for you to provide the names and contact details of any additional authorised family members.

We must also be aware of any medical conditions that your daughter may have - it is extremely important to give as much detail and information as possible. Failing to provide us with the correct and most up-to-date information on your daughter’s health may put your daughter at risk.
