Please complete this form and submit by July 15th. Applications will be screened and new mentors will be interviewed by phone by the Mentoring Program Committee by August 15th.

Final circle assignments will be announced at the Mentor Orientation which will take place on Zoom  on September 2nd, 6-8pm.

The Program Kick-Off Event will take place on Zoom on September 29th, 6-8pm. Attendance at this event is mandatory for participants.
