As you are aware, Ontario Presents is on a continuing journey to incorporate equity, diversity, inclusion, accessibility, and decolonization (EDIAD) into all that we are as an arts-service organization. To do this work is to also learn how to support our members on their own EDIAD journeys. 

If you could take some time to provide feedback through this survey, you will be helping us establish a baseline identifying where our members are on their EDIAD journey. With this information, we will then determine more opportunities for weaving EDIAD into all of our programs and services that can best support our members. 

The survey should take about 10 - 20 minutes to complete. Your responses will remain anonymous and only be shared with Ontario Presents staff. 

Only one response per member organization is requested.
