If you have been involved or witnessed an incident during Fantastic Arcade that you believe violates our Code of Conduct (fantasticarcade.com/forms/coc), use this form to report the incident. You may do so anonymously but if you would like the Community and Administrative Board to investigate and follow up, please let us know how we can contact you. This form will be submitted to the Community and Administrative Board at Fantastic Arcade. If you would like to reach out to a third party response team you can do so here (link here).

The Board will attempt to reach out to the reporting party before taking any action, unless the situation requires quicker action for the safety of the entire community.

Describe the incident in as much detail as possible: time and place where it occurred, who was involved, did others witnessed the incident, etc. Try to be as specific as possible:
