Please complete this form if you are interested in enrolling your student in the PISD Private Lesson Program for the 2022/2023 school year.  Lessons are $22 for a 25-minute lesson once a week during your child's orchestra class period. Please be sure to fill in all information that pertains to you, including teacher requests (if applicable).  It is important for us to start building a lesson schedule now so that private teachers can plan accordingly and lessons can begin promptly in August. If you submit this form and then change your mind at any point this summer, it's not a problem! Just please email me and let me know.
