Human Health Passport 1.1
The Human Health Passport was developed by the Institute of Gerontology of the National Academy of Medical Sciences of Ukraine with the purpose of screening population’s health, working capacity, and biological (functional) age of the person and employee, and identifying the preventive measures needed to maintain the health and efficiency of the population.
 Only the results of this survey were used in this study. Each respondent was given a code that matched the time of the survey. The email addresses were only used to automatically send the results via Google Form. The scores mean: 20-21 points - "you are completely healthy, physical and deep breathing training", 19-17points -  "you need to harmonize the optimal relationship between rest and work, physical training and deep breathing exercises" , 15-16points - "you need to pay especially attention to physical activity, deep breathing exercises and detail consultation by physician" , 14-7 points - "you need the active rehabilitation and diagnosis of your health, light physical activity ", 6 points and less -"you need  medical and social help, passive moving" … https://youtu.be/QIHT6_fy-jU  Somewhat simplified scales are used worldwide for definition and assessment of senile infirmity, internal capacity, and assessment of cognitive abilities. We have developed a Human Health Passport in a pilot mode using the available free services. The content of the Human Health Passport takes into account the attainments of the Institute of Gerontology of Ukraine, world-class gerontologists from USA, France, India, Canada, Australia and etc., as well as WHO recommendations. Such testing helps a person pay attention to their health and ability to work, apply prophylactic measures to eliminate disharmony, maintain and successfully achieve personal creative potential, and enhance a healthy aging. By completing this form of research, you consent to the processing of your anonymous results to develop preventive measures and treatments to improve your health. The results of the cohort's data are used in the scientific innovation for preventive measures to improve and maintain human health in the COVID pandemic crisis.
