Parents/Guardians: In an effort to protect the health of your child, other students, and our staff, we are requiring daily health screenings for our students.  It is the expectation that you will conduct this daily check for symptoms each morning before allowing your child to go to school.  


We are collecting this one-time sign-off from our parents/guardians as your confirmation that you will complete this monitoring on a daily basis and assure Woodbury City Public Schools that your child will not attend school on the day these symptoms are present or has had any exposure to Covid-19.
