Welcome to the American Chemical Society (ACS) Cleveland Section's celebration of National Chemistry Week (NCW)!  Due to COVID-19, we are celebrating NCW this year with an online chemistry contest!  Three prizes of $50, $25 and $25 will be awarded to the first, second and third place winners in each of two age groups:  grades 2-3 and 4-6.  In the event of "tie" submissions, random drawings will be performed.  Parent/guardians of cash prize winners will be contacted via e-mail after the end of the contest for mailing addresses.  

New update - we have extended the submission window till 10/31!   To complete your entry, please answer ALL of the following questions.  Submissions must be received by 10/31 (new!) to be eligible for entry and/or cash prizes.  

All COMPLETED submissions will receive an entry prize WHILE SUPPLIES LAST.  You will receive a confirmation e-mail once your form has been accepted.  The first 3 COMPLETE submissions received at each Cuyahoga County Public Library branch (based on date/timestamp) will receive a special entry prize.  Entry prizes may be picked up during the contest submission period (10/9-10/31 - new!).  In the event more students enter than prizes are available, prizes will be awarded according to earliest submission/timestamp.  

Please bring a COPY of your entry, which you will receive at the e-mail address you list below, to the library branch you select ON THIS FORM when you pick up your prize.  Entry prizes are generally small in size and may present a choking or other hazard, so children should be supervised with their prizes.  

An adult (parent/guardian) must be the point of contact.  Entries from schools/class entities will not be accepted, but teachers are encouraged to suggest their students enter individually.  Contact information will only be used for the purposes of this contest.
