•  Class location is 13 West Market Street, Blairsville, PA 15717.

About the Class
•  This class includes instruction on Beginner Glass Crafting: Learning the basics of glass crafting, glass cutting, glass colors, and students will cut/design their project. Instructor will then kiln their project off site and bring back for pickup.

Class Information:
•  The class is for students ages 13+
•  The cost of tuition is $35 per student and includes all materials.

Class Dates:
•  April 11 from 6-8pm for Teen Glass Crafting (ages 13-18 - adult must be present)
•  April 18 from 6-8pm for Adult Glass Crafting (above 18)

Additional Details:
•  There will be a class maximum of 8 students (with a minimum of 4 students required to start)
•  After class, projects will be taken with instructor to fire in the kiln and will be available for pickup on April 25 at 6pm.  (If you are unable to pickup during these times, please coordinate with Charlotte Robertson - thestaffmusicstudio@gmail.com or call/text (724) 549-2368)

Tuition Policy - 

Payment may be remitted through this form, sent via Venmo @charpip24, cash/check deposited into the tuition box at the studio. 

Tuition must be received along with this completed form to hold your spot.

Which class will you be attending? *
Attendance Policy - 

No make ups will be offered/tuition refunded for classes missed by students.  In the event that the instructor has to miss a class, a make up will be offered.  If the student is unable to attend the make up class offered by the instructor, tuition will not be refunded/adjusted.

PHOTO/MEDIA RELEASE - 

1. Charlotte Robertson DBA Staff Studio and other Staff Studio instructors are permitted to take photos/videos of and use photos/videos of student for the purpose of education/advertisement (internet and print) with no compensation to the student/parent/guardian.

MEDICAL RELEASE - 

1. Staff Studio carries an insurance policy for the safety of our students and instructors, HOWEVER, in the unfortunate event that a student would require medical attention (ambulance, hospital visit, etc.) Charlotte Robertson DBA Staff Studio is released of all monetary/legal responsibility and/or liability.  

2. Please be sure that your student is aware that unsafe practices (running, jumping, rocking on piano benches/chairs, etc) are not suggested and if they result in an injury, Charlotte Robertson DBA Staff Studio is released of any and all monetary/legal responsibility and/or liability.  

3. Additionally, any and all health/legal risks associated with COVID are the responsibility of the parent/guardian listed on this form.  We do not require masks for our students or instructors.

COMMUNICATION POLICY - 

1. Generally speaking, studio-wide communication will be received from Staff Studio via email. Your student's individual instructor may choose to communicate via phone call, text, email, etc.  
      a. Please note that emails from Staff Studio will           come from either thestaffmusicstudio@gmail.com or via My Music Staff.  
      b. Please be sure to allow emails to go to your regular email and not your junk mail so that you get any and all pertinent emails.  
      c. The email provided on this form is the email that will be utilized for communication.  

2. Please check your email periodically and before class for studio updates. 

3. We are not responsible for communication missed by the student.

REFUND POLICY - 

100% of tuition paid will be refunded if request is made more than 15 days before the start of class.  50% of tuition paid will refunded if request is made between 7 - 14 days before the start of class.  No refunds will be made within 7 days of the start of class.
