By submitting this form I declare that: 1) I fully understand, support, comply with, and abide by the Ontario Badminton Academy (“the OBA”) all its terms and conditions, rules and regulations, decisions, announcements, signs, indications, notifications, either they are currently existed or going to be posted later on, either they are posted on its walls, website, or mailed, emailed, texted, or told by its staffs, coaches, officers, and/or people that working for it (“the Agents”); 2) I hereby assume all risks and my responsibilities for all activities at “the OBA” premise for my child/children and myself as well as my accompanies (“my People”), and I agree that I give up the right to sue “the OBA” and release “the OBA” and “the Agents” forever from any and all claims for personal injuries, death, property damages or losses happened due to “my People” participation in the activities at or related to “the OBA”; 3) I give my consent to the use by “the OBA” of photographs and videos of “my People” for badminton  promotional purposes; 4) In the event that I am responsible for any damages or losses of the equipment or facilities of “the OBA”, I agree to make immediate payment for the cost of each incident.

All campers using the courts must wear NON-MARKING shoes.  Campers who enter the court area without proper shoes will be asked to leave with no refunds.

I agree and give permission for the Ontario Badminton Academy to record, film, photograph, audiotape or videotape my child’s name, image, and activities and to display, publish, or distribute these depictions for the purpose of publishing, posting on the OBA website, in the Club, and posting of social media sites.  I hereby waive the right to approve the use of these depictions now or in the future, whether the user is known or unknown, and I waive to approve the use of these depictions.  I understand that the depictions may appear in electronic form on the internet or in other publications outside of the Club’s control.  I agree that I will not hold the OBA responsible for any harm that may arise from such unauthorized reproduction.

I agree and acknowledge OBA is not liable for any accidents, bodily harm, or loss of life of the child if the child leaves the gym during or after camp by himself/herself.

All payment has no refund, no make up classes, no credit
$10 administration fee will be applied for each schedule change for each single student
