Elite Studio Registration 2021-2022

Please fill out 1 registration form for each student that you are registering and check all of the boxes that apply for the classes they are taking.

Once we receive your registration, we will email you a class confirmation, a tuition schedule, a handbook, and a Fall flower fundraiser form. Forms are due by August 23rd. Please write one check to "Elite." Someone will be at the studio from 7 to 8 on August 23rd for you to drop forms off. Pick up will be on September 15th.

Classes that are full will be listed as such, if that occurs.

Social Media Release (Please type your name below as a form of a digital signature, if you consent to the Social Media release) I understand that from time to time Elite may use class or student photos. I give them permission to use my child's photo on Facebook, the website, in the newspaper, or other forms of Social Media.

Liability Release (Please type your name below as a form of a digital signature, if you consent to the Liability release. All students enrolled must have a liability release on file before participating in classes at Elite.) I understand that Elite and its instructors are not liable for personal injuries or loss of or damage to personal property. Since dance is a physical activity, injuries may occur. Each student may decline to participate in any activity which the student deems harmful and must inform the instructor of any physical limitations which may prevent full participation in class.
