This form must be completed before granted guest access to the building. Anyone with COVID symptoms are not permitted to enter the building. If any of the indicators are positive stay home until you meet the following criteria: or isolate for 10 days and have no symptoms for the past 3 consecutive days. Note: A positive COVID test result requires a 10 day isolation and 3 days symptom free including fever for 3 consecutive days before returning to school.
