Participation Waiver - I am fully aware of, and appreciate, the risks, including the risk of serious injury, as well as other damages and losses, associated with participation in lacrosse practices, games, and other similar events (“Lacrosse Events”).  I agree on behalf of myself, my heirs and personal representatives, that Ridgefield HS Boys Lacrosse, Ridgefield Public Schools, and the Town of Ridgefield, as well as agents, officers and directors of those organizations, with respect to these Lacrosse Events, together with any coaches, officials, volunteers, and employees working in connection with these lacrosse events shall not be held liable for any injury, loss of life or other loss or damage as a result of my participation in these Lacrosse Events.  I hereby give my consent to the host organization to provide through a medical staff of its choice, customary medical/athletic training attention, transportation and emergency services as warranted in the course of my participation in these Lacrosse Events. I will only participate in the event that I a physically and psychologically prepared to compete. *

COVID Waiver- I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that RHS Boys Lacrosse has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.I further acknowledge that RHS Boys Lacrosse can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, lacrosse staff, and other lacrosse players, lacrosse staff and their families. I voluntarily seek services provided by RHS Boys Lacrosse and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending these lacrosse related events.I attest that:* I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.* I have not traveled internationally within the last 14 days.* I have not traveled to a highly impacted area within the United States of America in the last 14 days.* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non-contagious by state or local public health authorities.* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.I hereby release and agree to hold RHS Boys Lacrosse harmless from and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses, and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the lacrosse staff, or that may otherwise arise in any way in connection with any services received from RHS Boys Lacrosse. I understand that this release discharges RHS Boys Lacrosse from any liability or claim that I, my heirs, or any personal representatives may have against the basketball staff with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from RHS Boys Lacrosse. This liability waiver and release extends to the entire lacrosse staff, volunteers and lacrosse board.COVID-19 is an extremely contagious virus that spreads easily through person-to-person contact. Federal authorities and the State of Connecticut recommend social distancing to prevent the spread of COVID-19. Contracting COVID-19 can lead to severe illness, and in some cases, death. Use of the Ridgefield Parks and Recreation (RP&R) and Tiger hollow facilities could increase the risk of you or your child contracting COVID-19. RP&R, Tiger Hollow and RHS Boys Lacrosse, as well as it's staff, undertake every possible effort to keep our facility clean and disinfected, as well as mandate social distancing at all possible times. However, as with any public facility, we can not guarantee that you will be 100% safe from airborne illnesses such as COVID-19, the flu, etc. Signature of this waiver, confirms that you accept these risks and will not hold RP&R, Tiger Hollow  or RHS Boys Lacrosse responsible in the case of someone in your household contracting COVID-19 or any other illness. *
