ISLA Tennessen Warning for Student Application Form
1. ISLA is asking you to provide information that includes private and/or confidential information as classified by Minnesota law. ISLA is asking for this private information so that ISLA can enroll your child
2. You are not legally required to provide the information ISLA is requesting and you may refuse to provide some or all of the information requested.
3. If you provide this information ISLA will evaluate and use it for the purposes noted above, and may accordingly grant the privilege of enrollment at ISLA
4. If you do not provide sufficient information ISLA may not be able to enroll your child at ISLA.
5. With some exceptions, unless you consent to further release of private information, access to this information will be limited to individuals employed by ISLA who have a need to know the information in order to provide services to you or to the ISLA-PTO to offer opportunities to be involved in the ISLA community. However, state law does authorize release of private information without your consent to:
•
a court, grand jury, or state or federal agency, if the information is sought with a subpoena;
•
appropriate persons in connection with an emergency, if necessary to protect your health or safety or the health or safety of others;
•
if required by a court order, or permitted by other state or federal law.

1. ISLA le pide que proporcione información que incluye información privada y/o confidencial según lo clasificado por la ley de Minnesota. ISLA solicita esta información privada para que ISLA pueda inscribir a su hijo
