2023 Tribal Elder Food Box Survey
This survey is for individuals who have received a Tribal Elder Food Box between May 2023 and today. 

We value YOU as partners and desire to serve you and your community’s food needs better. 

Feedback will be used to make improvements to the Tribal Elder Food Box Program and to secure funding to continue the program next year. 

We would appreciate all responses by December 31, 2023. You may return your completed survey in-person at your distribution site. You may also submit your completed survey by mail to: Feeding America Eastern Wisconsin, 2911 W Evergreen Dr. Appleton, WI 54913.

Current distributions are May-December. As we invest in Indigenous food production, we are trying to determine whether to prioritize funding to create a winter growing season (greenhouses), to build commercial kitchens to dehydrate and can locally grown fruits, vegetables, meats, grains, etc, or other.
