THE MONOLOGUE SHOW: Parent Permission Form
(This form is to be completed by the parent/guardian.)

In an effort to ensure that both students and their parent/guardian(s) are fully aware of the commitment being made by students who participate in Dixon-Smith Middle School Theater productions, we ask that the parents/guardian review the content of this form with their student initialing and signing each section.,

This form must be submitted by March 7 at 5pm in order for a student to be considered for casting.


REGULAR REHEARSALS:
Rehearsals begin for the full cast on Wednesday March 10, 2021 from 9:00 AM-10:15 AM
(students may attend in-person or virtually)

BEFORE-SCHOOL REHEARSALS:
Rehearsal will take place Tuesday– Friday from 9:00 AM - 10:15 AM

NON-SCHOOL DAY REHEARSALS
MONDAY, April 19 Is an Asynchronous in-person rehearsal. We will record on this day.

SPRING BREAK: There will be no mandatory rehearsal over spring break, but students will be expected to work on their part and may be asked to submit performance videos over this time. There may be some voluntary rehearsals scheduled as well.

PERFORMANCE RELEASED FOR ON-DEMAND VIEWING:
THE PERFORMANCES WILL AVAILABLE FOR  STREAMING APRIL 23-26

As the parent/guardian, I understand that if my student is cast or joins the crew, it will include committing to the schedule detailed above. *

I understand that we, the student and parent/guardian, can submit conflicts with the above schedule on this form, and that they will be taken into consideration in the casting and crew staffing of the play. Also, conflicts added after the play is cast/staffed may result in the student being removed from their role in the production. Should unexpected conflicts arise, we will notify the director as soon as possible so that they can be taken into consideration. *
Conflicts with the Schedule:
As the parent/guardian, I understand that this is a virtual production, and therefore it will be recorded and released online for public viewing. Also, this production may include costumes, minor set pieces, props and portable equipment being provided to students in order to execute the production, and that these items might be in our home. *
