AKPC Church Site Small Gathering Report Form (교회 소그룹 모임 신고 양식)
Any group having a small group gathering at the church site (either indoor or outdoor), please enter the information for the following questions at least 24 hrs prior to the meeting. You should fill out a new form for each meeting. / 교회 건물 내부 혹은 외부 장소에서 소그룹 모임을 가지는 분들은 적어도 모임 예정 시간 24시간 전에 이 양식을 완성해서 제출해주세요. 매번 모임마다 새로운 양식을 제출해주셔야 합니다.

Your answers will be kept confidentially and will not be shared with others, except when asked by the health authority / 여러분이 제출한 개인 정보는 기밀자료로 관리되며, 지역 보건기관의 공식 요청의 경우를 제외한 다른 개인이나 기관에 공개되지 않습니다.

Leader must keep a list of the participants with their contact information, and should contact the Health Task Force (elder Dong-Ha Min, dongha.min@gmail.com) immediately if there was any case or close contact case known from the gathering. / 모임의 리더는 참가자들의 명단과 연락처를 가지고 계셔야합니다. 만일 모임 참가자 중 감염자가 생기거나 감염자와 접촉한 이가 생기면 교회 Health Task Force (민동하 장로, dongha.min@gmail.com)에 즉시 통보하셔야 합니다.
