I hereby understand that if this is an outdoor event, there will be protection from the sun, wind, and precipitation. *
I hereby understand that it is my responsibility to ensure that the required equipment is provided at the ceremony venue for the performance. Required equipment includes: sound amplification system with a microphone and available acoustic guitar jack (1/4" or XLR); a boom style mic stand; a sturdy music stand.                                                                                                                                                  If the church/venue does not have an amplification system, one can be provided for a $35 fee.  A power source is required. *
I understand that for a liturgical celebration, MIGUEL OCHOA will provide acoustic guitar and/or vocal music for the following selections: Entrance Hymn, Offertory Hymn, Communion Hymn, Recessional Hymn, and all mass parts (Gospel Acclamation, Holy Holy, Memorial Acclamation, Great Amen, Agnus Dei).  Optional selections include: Seating of Parents, Bridal Party Entrance, Responsorial Psalm, Unity Candle Lighting (or other symbolic ritual), and a Hymn for the Presentation of Flowers (or Communion Meditation).  I understand that I am responsible for selecting the aforementioned hymns and ensuring their suitability with the celebrant/parish expectations.   *

I hereby understand that if I wish to have a musician join MIGUEL OCHOA in providing music for the event, at least one rehearsal is required and I will be charged an additional fee of $35 per hour for rehearsal time. *

I hereby understand that MIGUEL OCHOA is not required to attend the wedding/quinceañera rehearsal unless otherwise discussed.  Attendance at the rehearsal will incur an addition $25 fee. *

I hereby understand that if the aforementioned event is canceled, I must directly contact MIGUEL OCHOA at least 30 days prior to the event. If the performance is cancelled within 30 days of the event, I will be charged a $100 cancellation fee which is due within 14 days of cancellation. *
Performance attire includes slacks, a button up shirt, belt, and dress shoes.  A coat and/or bow tie will be worn upon request.  Please feel free to list any specific attire requests here:
I understand that by submitting this contract the CLIENT listed above is committing to booking MIGUEL OCHOA for services at the aforementioned event.  I understand that payment for the event must be submitted to MIGUEL OCHOA at least 24 hours before the services are to be rendered via VENMO (@Migo8a). *
