Please fill out a separate form for each student you plan to enroll. Then, fill out and submit a "Request for Student Records" form to your student's previous school, if enrolling in grades 1-10.

Once your application has been received, the MCA principal will reach out to you to set up a personal education planning meeting with your family.

Enrollment Process

Phase 1:
        Complete New Student Application for Admission (online)
        Schedule and attend personal education conversation meeting with the principal
        Submit Early Childhood Screening results (for pre-k and kindergarten)
        Please complete screening through your local school district
        Request for Student Records (Grades 1-10)

Phase 2:

Fill out online application agreements form

Pay "Hold My Seat" fee ($150 by March 1, +$50 each month)
         (Please make check payable to Minnetonka Christian Academy or pay online)

Fill out online financial scholarship form (if applying for aid)

All documents due before first day of school:
    Doctor Health Examination form (for K, 5th & 7th grades) to be completed by physician
    Copy of Birth Certificate or proof of age
    Copy of Immunization Records
    Copy of Health Insurance card
