Please read over the device agreement carefully and indicate below that you understand and agree to the terms and conditions. NOTE: All K-1st grade students are receiving a pre-loaded iPad, and all 2nd -8th grade students are receiving Chromebooks.  ALL students must have this device agreement on file.

Student Section: In addition to the following terms, the ADHUS/FAU High School Computer Agreement and Technology Policy* also applies. I agree to take care of the device while it is checked out to me. I agree to immediately notify my teacher of any damages whether accidental or intentional so my device can be turned in for repair. I agree to return the device, in good condition, to ADHUS/FAUHS on the date to be determined by ADHUS administration.

I will not...

☐     Throw, drop, or damage the device in any way  
☐     Give the device to another student for his/her use
☐     Download, purchase, or change the content loaded on the device
☐     Eat or drink around the device
☐     Put anything on top of the device


ADHUS/FAU High School: Parent Device Agreement

I am authorizing the use of a school-issued iPad or Chromebook for my child. I understand that it is to be used as a tool for learning and that my child will comply with the ADHUS/FAU High School iPad/Chromebook Agreement and Technology Policy* as well as this Device User Agreement. I will ensure the safe and timely return of the device when regular classes resume or the return date designated by ADHUS Administration.

I understand that I am financially responsible if the device and accessories are stolen, or for any willful, malicious, or accidental damage to the device, which includes any charges resulting from unauthorized content downloaded. I also understand that within 48 hours upon receiving the device, that I will notify ADHUS/FAUHS administrators of any missing accessories and if the device itself is malfunctioning or damaged.

The replacement cost of the devices: iPad: $250 (LCD screen $100/AC Adapter $15/cable $10), Chromebook 3: $150 (LCD screen $60/charger $20), Chromebook Plus: $450 (LCD Screen $240/charger $25), Dell Chromebook: $311 (LCD screen $100/charger $25), Dell E550: $800
