Request for Intra-District Transfer/ Placement

Please use this form to request your child(ren) to attend an elementary school other than their assigned school. Assigned schools are determined based on approved boundaries, which can be found on our website at: https://www.sdoj.org/district/boundaries.cfm and by reviewing Administrative Guideline ag5120: Assignment to School, Class, and Grade.

IMPORTANT INFORMATION:

  • If you have moved out of the School District of Jefferson, do not complete this form. A separate form is required. Please contact your school office or the District’s Open Enrollment Coordinator for more information.
  • Approval is granted on an annual basis based on space availability. Reapplication is not required each year.
  • Once a request is approved, the student shall remain in that elementary school until a new Request for Intra-District Transfer is submitted, or the student moves to the middle school.

INFORMACIÓN IMPORTANTE:

  • La aprobación se otorga anualmente según la disponibilidad de espacio. No es necesario aplicar de nuevo cada año.
  • Una vez que se apruebe una solicitud, el estudiante permanecerá en esa escuela primaria hasta que se presente una nueva solicitud de transferencia dentro del distrito o hasta que el estudiante se transfiere a la escuela intermedia.
