Application for receiving a Stephen Minister
Stephen Ministers are congregation members who have been trained to offer care to people who are hurting. A Stephen Minister meets in person with the Care Receiver needing care once a week for about an hour, for as many weeks as the Care Receiver and Stephen Minister determine. 

To receive care from a Stephen Minister, please complete the below application. Then we will contact you for an in-person interview with either a Pastor or Staff Member. If situation is appropriate for a Stephen Minister one will be assigned. If not, alternative resources will be communicated and provided.
