Healthy Franklin County - Stories of Success – Entry Submission

Healthy Franklin County is coordinating the Franklin County Health Summit on October 20th, 2022, which will include the release of WellSpan Health’s Community Health Needs Assessment (CHNA) highlighting County-level data. The CHNA helps our community partners address identified health priorities. Healthy Franklin County is in the process of planning the health summit which will feature short stories that highlight the impact of the services or programs you provide to your clients or consumers. More than one story per organization is acceptable but please submit each story separately. Please submit this form no later than September 16th, 2022. Contact npurdy@wellspan.org with questions. If your story is selected, you will be asked to identify someone to read the story at the health summit on October 20th.

Story Submission Focus Area (Please check all that apply as many will intersect with another).

Mental Health: Mental health includes our emotional, psychological, and social well-being. It affects how we think, feel, and act. It also helps determine how we handle stress, relate to others, and make choices. Mental health is important at every stage of life, from childhood and adolescence through adulthood.

Food Security: Food security, as defined by the United Nations' Committee on World Food Security, means that all people, at all times, have physical, social, and economic access to sufficient, safe, and nutritious food that meets their food preferences and dietary needs for an active and healthy life. The goal of promoting healthful diets and healthy weight encompasses increasing household food security and eliminating hunger.

Nutrition and Physical Activity: Good nutrition, physical activity, and a healthy body weight are essential parts of a person’s overall health and well-being. Together, these can help decrease a person’s risk of developing serious health conditions, such as high blood pressure, high cholesterol, diabetes, heart disease, stroke, and cancer. A healthful diet, regular physical activity, and achieving and maintaining a healthy weight also are paramount to managing health conditions so they do not worsen over time.

Early Childhood Education & Development: Early childhood, particularly the first 5 years of life, impacts long–term social, cognitive, emotional, and physical development. Healthy development in early childhood helps prepare children for the educational experiences of kindergarten and beyond. Early childhood development and education opportunities are affected by various environmental and social factors, including: early life stress; socioeconomic status; relationships with parents and caregivers; access to early education programs.Option 4

Social Determinants of Health: Social determinants of health (SDOH) are the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. The five domains of SDOH include economic stability, education access and quality, health care access and quality, neighborhood and built environment, social and community context. Specific examples of SDOH include: safe housing, transportation, and neighborhoods; Racism, discrimination, and violence; education, job opportunities, and income; access to nutritious foods and physical activity opportunities; polluted air and water; language and literacy skills.Option 5
