Dear Parents,

Due to the COVID-19 pandemic we will have to modify our CCD program this year. In an effort to effectively and safely provide instruction for our CCD students and offer options to our families, we ask that you please take a moment to fill out this short survey to aid us in the planning of our faith formation program for the 2020-2021 Catechetical year.

We will continue to monitor changes in COVID-19 cases and adjust our plans immediately to ensure the safety of our families.

I prefer my child to have socially distanced in-class faith formation. (Classroom size will be kept to a minimum and children will be socially distanced 6 feet or more, hand sanitizer available in every classroom, classrooms will be sanitized). *
