I'd love to know more about your journey and your goals as a singer.  What did you grow up singing and what do you primarily sing now?  
Have you had voice lessons in the past and had success? What goals have been met and what are you working toward still? Please tell me about your journey through working with your past teacher or coach if you've had one.  If you never have, that's a perfectly acceptable answer.
Do you have any vocal health concerns or have you in the past?  Have you ever seen an ENT or laryngologist and been scoped?  If so, do you have the photos/video available? Will you please bring them?
Are you on any medications that cause dryness? For example, do you take something like Sudafed regularly? If you are willing, please share any other medical issues so I can be aware of how your body works!
What would you like to know about me or studying with me?
Would you be willing to bring something to sing for our first lesson?  Show me what your voice does.  What are you used to singing? It can be simple like a folk song or a hymn.  It certainly doesn't have to be classical, but it can be. You may bring an accompaniment/karaoke track if you wish (if online, have cued on a separate device other than the one you're using for lessons).  You may also sing it a cappella or play along with yourself on your instrument.

Please also bring headphones (open back preferred) or ear buds to your first lesson. Ethernet connection is preferred over wifi. If you don't have either of these, don't worry! Have you read/followed these steps?
