GMHS 2020-2021 Athletic Waiver
As the parent/legal guardian, I authorize my child's full participation in spring sports at GMHS.  I understand that injuries, severe, minor, permanent damage or even death can occur as a result of participation in this program including the risk of contracting COVID 19.  In consideration of my child being allowed to participate, I, myself, my child and covenant not to sue, Georgetown School District, the Town of Georgetown and all their staff/employees/volunteers for any injury, illness, loss or damage resulting or arising from my child's participation in the program and/or while upon the premises where the activity is being conducted even if caused by negligence of the District, Town or their staff/employees/volunteers.  My child is covered with family insurance in the event of an accident or injury.  I attest my child is in good heath and has no restriction on participating in athletics.
By agreeing, you have read and understand all the rules and information presented in the handbook, and that you agree to abide by the rules set forth and are willing to face the consequences if you choose to violate them.  You also acknowledge that you have read and will abide by the Hazing regulation, read and agreed to the State Law Regarding Sports-Related Head Injury and Concussions and completed one of the online courses per the requirements of this law.

Concussions: Students and Parent Information
Parents and students - athletes who plan to participate in any Georgetown Middle High School athletic program must complete the free on-line concussion course.  This on-line course contains all information required by law.  The entire course, including registration, can be completed in less than 30 minutes. The program is available at https://nfhslearn.com/courses/concussion-in-sports-2.  Both parent/guardian and student athlete have taken the online course regarding head injuries and are aware of the recent changes/updates to the head injury legislation.  Georgetown Middle High School uses return to play protocol in situations where head injury is concerned.

Information on Substance Use Disorder Prevention
Recent Massachusetts legislation requires that parents, guardians and students in 6th-12th grade receive educational materials on the dangers of opioid use and misuse.  The educational information must be distributed to students involved in an extracurricular athletic activity prior to the beginning of their athletic seasons.  The Massachusetts Department of Health with the Massachusetts Interscholastic Athletic Association (MIAA) and Massachusetts Technical Assistance Partnership for Prevention MAssTAP) have developed a set of four fact sheets for the extracurricular school sports community on prescription opioid misuse prevention entitled:  What to Know About Prescription Opioid, Preventing Prescription Opioid Misuse Among Athletes, Guidance on Communication After Opioid Non-Sports Injury, and Injury Management Opioid Misuse Prevention.  Links for each of these can be found at:  http://www.mass.gov/eohhs/gov/departments/dph/programs/community-health/dvip/injury-prevention/substance-use-disorder.html  My student-athlete and I have reviewed this information prior to the start of the athletic season.

Spring Sports Modifications Related to COVID 19
Due to the current public health situation, the state of Massachusetts along with the MIAA have created sport specific modifications for all fall II sports for the 2020-21 season.  These can be viewed here: http://miaa.net/contentm/easy_pages/view.php?sid=38&page_id=317.  My student-athlete and I have reviewed this info prior to the start of the athletic season.  User fees are non-refundable if a student-athlete quits after making the team or if the program is shut down due to COVID-19.
