Your application will not be considered until this form, along with the $500 deposit, are received. Upon review of these two requirements, you will be notified as soon as possible regarding your acceptance, as well as further planning details. This trip, offered by Alpha Climbing LLC, is intended for experienced youth athletes. To the best of my knowledge, the applicant's ability meets the minimum level required for participation.

I recognize that performance level rock climbing involves risk, and that by choosing to participate in this program, I voluntarily accept the risks involved.

I accept responsibility for documentation required for travelling internationally, including, but not limited to: current passport, entry visas, travel affidavit for a minor, insurance documentation, et al.

I understand that departing the trip before the end date shall not be grounds for a refund of any amount.

I understand that the itinerary of this program is built on rock climbing, and that a minimal amount of sight seeing will occur.
