Activity Participation Agreement

Please complete a SEPARATE form for each participant.
Completion of the form is required for FUMC Huntsville Youth & First Kids to be able to participate in field trips and overnight events.
This must be completed every year for events, field trips, and overnight trips with the church.
Parental Consent and Medical Release for activities during the 2024 year.
It is the parent's responsibilty to notify the Youth Director should information on this form change and need to be updated.
