Welcome to the second part of applying for the CYC Ministries LIT Program, we are excited that you are applying for LIT!

This form is designed to ask you a little bit more about you, as you apply to be a part of LIT.

As part of this form you will also be asked you to provide two references for us to contact, one PERSONAL REFERENCE (a teacher, or a family friend, or someone from church) and one PASTORAL REFERENCE (a Pastor from your church, your Youth Minister or other key leader from the church that you attend).

This form should be entirely filled out by the young person applying to be a part of LIT, as it is important for us to get an understanding from them as to why they would like to be a part of LIT.

Once this part of the application has been submitted, we will be in contact with both of your references, and follow up with you accordingly.
